For this assignment, you will now use the information and findings from your community needs assessment to complete the second phase of this project: development of a program/intervention implementation plan. Your program/intervention implementation plan will include: a description of the program, logic model outlining the intervention, implementation timeline and action plan, and a budget and budget justification for your intervention.
